Sacramental record request

To request a copy of your sacramental records please email or call the Parish Office with the following information:

  1. First and last name

  2. Type of record you are requesting: Baptism, Communion, Conformation, Marriage.

        *If you are requesting a Baptismal Certificate, did you have any other sacraments at St. Bernadette Church?

  If so, which ones?

  3.  Date of birth

  4.  Approximate date or year sacrament was made.

  5.  A phone number to reach you if there are any issues and to let you know it is ready to be picked up.

  6.  If you would like it mailed to you, the address it can be mailed.
